To enroll in Earl Warren, all incoming students need to PRINT and COMPLETE all the required forms indicated below.

If your student is not enrolling from a Solana Beach or Del Mar elementary school, please turn in all enrollment forms to the counseling office at Earl Warren. If you have any questions regarding enrollment, please call the registrar’s office at 858-755-1558 x4410. 

Please note: Earl Warren Middle School accepts intradistrict transfers.

Please follow the checklist so all forms will be submitted correctly and please remember to include:

• A copy of the student’s birth certificate
• Copies of 2 proofs of residency (one must be a current utility bill)
• A copy of a complete immunization record. Immunizations must include Hep B series, MMR, and proof of immunization or disease for chicken pox.  All dates must be signed or stamped by a doctor or clinic.
