1. Annual Notification Form
   State and Federal laws require school districts to notify parents and guardians of minor pupils of their parental rights.

• Parents are required
  to print out the last page of this form, sign and return to your school site in order for your student to attend school in fall 2005.

2. Emergency Form
   Required information for each and every students who attends school in this district.

• Parents are required
  to print out this form, fill in all the blanks, sign and return it to your school site, in order for your student to attend school in fall 2005. You must complete one Emergency Form for each student attending a district school.

3. Pesticide Procedures Notification
   Mandated by the California Healthy Schools Act 2000/AB2260.

• Parents are required
  only to read this form. 

4. Income Disclosure Qualification for Low Income Status
   Only for those families who wish to apply for free school bus transportation due to income status.

• Parents are required to:
  • complete and sign the form;
  • attach to a bus pass application gotten from your school;
  • return the form to your school site

5. National School Lunch Program
